  • Admission

    We accept referrals from physicians, social workers, nurses, discharge planners, other health care providers, and insurance providers or directly from patients and families. We will perform a thorough, pre-admission evaluation to determine whether your specific condition may benefit from our comprehensive rehabilitation program. The RehabCentre accepts most major insurance carriers, including Medicare and Priority Health.

  • Trained Staff

    Specially trained rehabilitation nurses provide quality nursing and medical care 24 hours a day. The rehabilitation nurse will educate you and your family on many topics related to your health and rehabilitation program, including medications and risk factors, as well as lifestyle modifications.

  • Unit Care

    The unit secretaries play a key role in your RehabCentre stay. They are responsible for making your stay seamless. They answer phone calls, put in orders for patient care, maintain patient charts and assist all staff members.

  • Daily Schedule

    A daily schedule board helps you keep track of sessions with nursing, physical therapy, occupational therapy, speech therapy, recreational therapy or other specialists, as well as any scheduled tests. Although your basic schedule will probably stay the same from day to day, there may be changes and adjustments.

    The RehabCentre provides 24/7 access to a physician or mid-level provider, and physicians are on site during the day. Our medical director, Pankaj Jandwani, M.D., is a board certified internal medicine specialist, with a special interest in physical medicine and rehabilitation. He is assisted by a mid-level provider. Together, they direct your overall care.

  • Education

    The RehabCentre was awarded a three year accreditation from the Commission on Accreditation of Rehabilitation Facilities (CARF), for its Stroke Specialty Program and Medical Rehabilitation Programs. Stroke patients benefit from weekly stroke education classes, designed to inform you of your risk factors, as well as how to manage your risk of a second stroke.

  • Meal Time

    Meals are served in the Rehab dining room in a friendly, social atmosphere. Mealtimes are 8 – 9 a.m., noon – 1 p.m. and 5 – 6 p.m. We strongly encourage you to come to the dining room for meals. Our patients enjoy the social interaction, and it is an important part of your rehabilitation journey.

  • Occupational Therapy

    Occupational therapists will evaluate your ability to perform tasks such as dressing, feeding yourself, hygiene, bathing, using the toilet, cooking, eating, cleaning and laundry. They will teach you how to perform these tasks safely with the least amount of energy and can recommend helpful equipment. We encourage your family to attend, so they understand your treatment program.

  • Physical Therapy

    Physical therapists work to maximize your independence. They help you strengthen and improve your ability to perform activities such as getting in and out of bed, transferring in and out of a wheelchair, standing and walking, balancing and managing stairs. We encourage your family to attend therapy sessions, to learn how to assist you with transfers and walking.

  • Traumatic Brain Injury

    The TBI (or traumatic brain injury) gym gives our therapists the option to treat head injury patients in a peaceful, quiet setting with less stimuli, allowing the patient to stay focused on their therapy.

  • Evaluation

    The psychologist performs assesses cognition and decision-making ability and may offer recommendations to the team on how to successfully work with you, focusing on safety and motivation. The psychologist may also help you and your family cope and adjust to your condition, and may recommend a plan for continuing care after you leave the RehabCentre.

  • Regaining your Lifestyle

    Our rehabilitation team shares a common goal of helping you regain your lifestyle. You and your family members are the center of the treatment team. Team meetings are held weekly to inform you and your family of your current status and to identify discharge plans/needs.

  • Speech Therapy

    Speech pathologists treat speech, language, cognitive and swallowing disorders. The speech therapist may recommend changes in your diet if you have trouble swallowing. They may also work with you on speaking clearly; understanding what is said to you, memory, attention and judgment. Your family is welcome to observe and participate in your speech therapy program.

  • Recreation

    MidMichigan Medical Center - Gratiot RehabCentre Recreation

    A recreation coordinator provides activities and entertainment that you may choose to participate in. Examples include pet therapy, card games, movies, crafts and Wii therapy. The recreational coordinator is also available for one-on-one visits and to help with community re-entry.

  • Access to Specialty Care

    One key feature of the RehabCentre is that patients do not need to leave the facility for access to specialty physicians, lab services, diagnostic imaging and other necessary testing. These services are available at MidMichigan Medical Center - Gratiot. If a medical problem arises at any time during your stay, immediate action will be taken to ensure your safety.

    Social workers will assist you and your family throughout your stay at the RehabCentre. They are available to help with everything from coping issues to coordinating financial resources and planning for discharge. You may contact the social worker at any time during your stay, if you have questions.

  • Setting Goals

    It is the goal of the RehabCentre for our patients to return home. Prior to discharge you may stay in the transitional living room. This room is set-up as a mini-apartment and is designed as a place to practice independent living. We encourage family involvement and overnight stay.

  • Making you Comfortable

    Unit assistants are here to make your stay as comfortable as possible. They can help with needs such as getting you a glass of water, adjusting your pillows or answering questions. The unit assistant might also assist you in the whirlpool room, which is available for therapeutic relaxation.

  • Working with Therapists Outside

    The courtyard provides an opportunity for you to work with therapists on regaining skills. There are a variety of outdoor surfaces for you to challenge your newly developed skills. The courtyard can also be used for recreational therapy and gardening. Many different plants and flowers create a peaceful place to enjoy the outdoors.

  • Weekend Passes

    A temporary leave for up to eight hours may enable you to practice new skills outside the hospital while attending a family event such as a birthday celebration, dinner at a restaurant or an evening drive. A pass is typically scheduled on a weekend day or evening just prior to discharge and after family training is completed.

  • Home Evaluations

    A home evaluation may be part of your program. An occupational and/or physical therapist will have you do the things you normally do at home and make recommendations for safety and equipment once you return home.

  • Going Home

    Upon discharge you will be provided with written information regarding home exercise programs, medications and home care recommendations. Follow-up appointments with physicians as well as continuing therapies will be arranged. Prior to discharge the staff will ensure that all of your needs have been met.
