UPDATE: Classes & Support Groups

For your safety during these uncertain times, some face-to-face classes and support groups may be cancelled or adapted to follow social distancing guidelines. In some cases, we may offer virtual alternatives. For example, we currently offer FREE online learning for all childbirth education classes. If you register for a class, we will notify you of any changes or cancellations using the email address or phone number you provide at registration. MidMichigan Health will offer a full refund if we cancel a class. In the event that you need to cancel, our cancellation and refund policy will apply.

Fees & Payment

MidMichigan’s specialized PAD Program is offering Free risk assessments for patients meeting high risk criteria. The goal of the Program is to promote community awareness to ensure early detection of the disease.

 

Description

FREE PAD Risk Assessment

Risk Assessment Testing Criteria

Patients that have two or more of the following risk factors are considered high risk for peripheral artery disease (PAD) and would be eligible for this free risk assessment:

  • Smoker
  • Diabetic
  • Chronic Kidney Disease
  • High Cholesterol and/or High Blood pressure
  • Age (greater risk over age 50)
  • Overweight and Inactive
  • Family history of Heart or Vascular Disease

Risk Assessment Features

  • Ankle Brachial Index test performed by a registered vascular technician (A copy of the test will be scanned into the EMR and given to the patient)
  • One-on-One PAD education provided by a registered nurse coordinator
  • Risk factor modification including smoking cessation if applicable
  • An educational PAD booklet

Refund Policy

If you are unable to attend an event for which you registered, we are happy to reschedule you for another available date, transfer your registration to another eligible person, or process a full refund. To receive a transfer or refund, you must call MidMichigan Health Line prior to the cancellation deadline for your event.
