FREE PAD Risk Assessment
Risk Assessment Testing Criteria
Patients that have two or more of the following risk factors are considered high risk for peripheral artery disease (PAD) and would be eligible for this free risk assessment:
- Smoker
- Diabetic
- Chronic Kidney Disease
- High Cholesterol and/or High Blood pressure
- Age (greater risk over age 50)
- Overweight and Inactive
- Family history of Heart or Vascular Disease
Risk Assessment Features
- Ankle Brachial Index test performed by a registered vascular technician (A copy of the test will be scanned into the EMR and given to the patient)
- One-on-One PAD education provided by a registered nurse coordinator
- Risk factor modification including smoking cessation if applicable
- An educational PAD booklet
