Clare County Community Health Needs Assessment Survey Now Open

MidMichigan Health is conducting a Community Health Needs Assessment (CHNA) in Clare County. Community members (18 years and older) are invited to take part in the online Community Health Perceptions Survey open now through Sunday, May 26.

“Every three years, MidMichigan Health conducts a Community Health Needs Assessment for each of MidMichigan’s six medical centers in order to identify the health and social needs affecting the community,” said Stephanie Leibfritz, R.N., M.S., community health manager, MidMichigan Health. “Feedback from community members received through the survey is vital in helping MidMichigan Health understand the pressing health needs and priorities from the communities’ perspective. The results of the survey will be used to develop the focus of the next community health improvement plan and other wellness activities.”

Completed results from the survey are expected in June 2019 and will be shared publicly on the MidMichigan website at www.midmichigan.org/chna.
