A DocuSign application packet will be delivered to the email address you have provided. The packet includes step-by-step instructions and a list of required supplemental documents. The most common items that may be required in addition to the application form include:
- Confirmation of medical school graduation
- Immunization records
- Copies of liability face sheets, board certifications and licenses
- Copy of military discharge papers, if applicable
- Listing of CME credits
A Box.com account will be created for you to easily upload any documents required for medical staff credentialing or provider enrollment. We will provide instructions for how to access this account.
A Central Verification Office credentialing specialist will provide personalized service to assist you with completion of your application.
Applicants to the medical staff are responsible for assuring all required information is received in order for the application to be deemed complete.
A Special Note for MidMichigan Health Employed Providers - If you are employed by MidMichigan Physicians Group, you will be contacted by our Provider Enrollment Department, which is a division of the Central Billing Office. They will help begin the insurance payer enrollment process. For additional information, please contact the enrollment specialist at (989) 633-8018.
Save the Date for Your Credentialing Interview, an interview with the MidMichigan Health Credentialing and Privileging Committee (MCPC) is a required in order for your application to be considered complete. You will be notified in advance of the date of your MCPC interview.
Interviews are held each month on the third Thursday beginning at 7:15 a.m. You will want to make sure to save this date on your calendar.
You will be scheduled for an interview at the location most convenient for you. Interviewing centers are located in Alma, Alpena, Ann Arbor, Clare, Gladwin, and Midland.
Telemedicine Providers
If you are providing telemedicine services remotely and will not be providing patient care on site in a MidMichigan Medical Center or outpatient clinic, you may be eligible to complete the interview requirement by viewing our credentialing video.
If you are eligible for this feature, the Central Verification Office will provide you with instructions.
