Three Midland-Area Physician Practices Receive PCMH Designation

Three Midland-area physician practices were recently designated as patient centered medical homes (PCMH) by Blue Cross Blue Shield of Michigan. Family Medicine Associates of Midland, P.C., Family Practice Center of MidMichigan Medical Center–Midland, and Midland Family Physicians, P.C., were among 11 practices associated with MidMichigan Health to earn this designation. These medical practices are committed to providing health care that is relationship-based with an orientation toward the whole person. 

Each PCMH provides comprehensive care for individuals and families. A PCMH is lead by a primary care physician (defined as an internal medicine physician, family medicine physician or pediatrician) and provides long-term coordination and management of each patient’s health across all settings.

“Significant effort was contributed by the providers and staff of these practices and we are pleased to see this work recognized,” said Dan Sorenson, M.D., medical director for MidMichigan Health Network. “Their leadership in being among the early adopters within our communities to pursue this recognition is valued. Patients will benefit from their commitment to proactively manage their care, engage them in self management and provide coordination of the services they require.” 

Physicians within the PCMH designated area practices include: Jerry Ferrell, M.D., James Frye, M.D., Jennifer Grossman, D.O., Hugo Juarbe, M.D., Timothy Kosinki, M.D., Jakub Malarz, M.D., and Craig Sonke, M.D., of Family Medicine Associates of  Midland, P.C.; Eric Becker, M.D., Gail Colby, M.D., William Dery, M.D., Yolanda Fong, M.D., Kelly Hill, M.D., Arturas Klugas, M.D., Egle Klugiene, M.D., and Sasha Savage, M.D., of Family Practice Center of MidMichigan Medical Center–Midland; and Jennifer Aloff, M.D., Lorenzo Berlanga, M.D., and Mark Ostahowski, M.D., of Midland Family Physicians.

As a designated PCMH practice, patients receive the same great health care with enhanced services such as: 

  • Individualized health goals with the physician and using a team-focused systematic approach to track appointments and ensure follow up on needed services.
  • Effective and timely follow-up with patients on their test results.
  • Active counseling, screening and education on preventative care.
  • Coordination of referrals to specialists and providing specialists with patient information needed for proper care, such as lab work and test results.
  • Self-management education and support to patients with chronic conditions.
  • Clear documentation outlining the roles and responsibilities of the patient and physician in the treatment and prevention of disease.
  • 24-hour patient access to a clinical decision-maker. Access may include extended office hours, telephone access and linkage to urgent care.  

Additional family practice, internal medicine and pediatric offices in our region will be nominated in November 2012.
